All players must fill out a North Texas Academy Player Registration Form, a Medical Release and provide a copy of the players Birth Certificate in order to register for academy league play.

North Texas Academy Player Registration - Please note that you must first register through your home association (Example: PYSA or GLASA), who will provide a home association registration number and sign the form.

Medical Release Form – A Parent or Legal Guardian should fill out this form.

Birth Certificate - You will also need to bring a copy of your child’s birth Certificate.

Please bring these signed forms to your team manager or coach.
